Consent Form, Release, & Liability Waiver – Ziplining Activities

Outdoor ziplining activities at RCSP/St. Patrick takes place under the guidance and direction of employees and/or volunteers from RCSP/the parish.

 

As parent and/or legal guardian, I remain legally responsible for any personal actions taken by my camper(s) (“participant”). On behalf of myself and the participant(s), I hereby fully release, and waive any and all claims against, St. Patrick Catholic Church, Rockville, Inc., RCSP, and the Roman Catholic Archdiocese of Washington (a corporation sole), as well as each of their directors, trustees, corporators, members, employees, agents, volunteers, chaperones, and representatives associated with the activity/event (collectively and individually, “Church
Parties”), as set forth below in more detail. I understand that ziplining may involve high-risk activities, with an increased risk of personal injury. I agree that it is my obligation to carefully consider the risks involved before signing. With appreciation of the dangers and risks associated with this activity and all related activities, on my own behalf and on behalf of the participant, I hereby fully and completely release and waive any and all claims, including—but not limited to—those for personal injury, death, or loss, that may arise against the Church Parties. On behalf of myself and on behalf of the participant, I hereby fully and completely release and waive any claims against the Church Parties relating to and/or arising out of the activity(ies), to the fullest extent allowed by law. I agree on behalf of myself and on behalf of the participant, and our heirs, successors, and assigns, to hold harmless and defend the Church Parties from any claim arising from or in connection with the participant attending the activity/event or in connection with any illness or injury (including death) or cost of medical treatment in connection therewith, and I agree to compensate the Church Parties for reasonable attorneys’ fees and expenses which may incur in any action brought against one or more of them as a result of such injury or damage.

By agreeing to the above, I represent and warrant that I am the parent or legal guardian of the participant and have authority to enter into this consent, release, and waiver.
